In order to participate in any off campus activies, two different forms must be filled out per youth.  The Permission Slip/Medical Release must also be notarized.  Please also make a copy of their insurance card, front and back, then turn into Terri Jo.


During the school year, families volunteer to cook meals for PSY.  Please bring or prepare onsite a meal for the youth group (about 20 people with all the youth and adult leaders, plus extra if your family is eating with us) and clean up the dishes and the kitchen afterwards.  It is also helpful if you set the tables for the youth so that we can be prepared to begin eating at 6:15 pm.  The Youth House has a complete set of dishes, flatware and basic cookware including a stove, microware, refrigerator and dishwasher.  The church provides beverages for the meal.  The following link takes you to available dates that run from September through December.  Call or email Terri Jo to see if the date you want to select is still available.  We will meet during the summer, but it will not be a regular schedule.
